Job summary

Suncrest Hospice is seeking a Registered Nurse (RN) to provide patient care across home, SNF, and assisted living settings. You will be part of an interdisciplinary team, offering psych-social assessments and interventions.

This role requires weekend coverage on an 8:00am–8:00pm schedule, with travel across the service area using reliable transportation. The role emphasizes teamwork, strong time management, and the ability to learn and use electronic medical records.

Qualifications

• Current RN licensure in good standing in state(s) of practice.
• Ability to collaborate daily with nurses, social workers, chaplains, volunteers and CNA leadership.
• Strong time management to manage visits, documentation, meetings and urgent needs.

Responsibilities

• Provide care and services for patients visiting their residences (homes, SNFs, assisted living).
• Contribute to the interdisciplinary team with psych-social assessments and interventions.
