Job summary

Suncrest Hospice is seeking a Registered Nurse to visit patients in private homes, SNFs, and assisted living facilities as part of an interdisciplinary team. You will perform psych-social assessments and coordinate with community resources to support patient needs.

The role may operate in Case Management, Admissions, or Resource positions, requiring travel across the service area and strong collaboration with social workers and other healthcare professionals.

Qualifications

• Active RN license in the state(s) of practice.
• Ability to collaborate daily with a diverse care team.
• Strong time management and prioritization skills.

Responsibilities

• Provide care and services to a group of patients, visiting them in private homes, SNFs, and assisted living facilities.
• Act as a contributing member of the interdisciplinary team, offering psych-social assessments and community resource knowledge.
• Serve in Case Management, Admissions, or Resource roles as part of the patient care team.
