LifeCare Memory Partners, Inc. seeks a Registered Nurse in Wilmington, NC for a Clinical Nurse Liaison role. You will educate, coordinate admissions, and support patients and families through the hospice process.

The position requires an accredited nursing degree, an active NC/SC license, and at least two years of experience with hospice or home care preferred. Public speaking and strong communication skills are essential.

Qualifications

• Graduate of an accredited school of nursing (AD, Diploma or BSN).
• Current license to practice nursing in NC or SC.
• Two years of experience; hospice or home care preferred.
• Experience in marketing, public affairs, community relations or hospital liaison nursing preferred.
• Strong communication skills in verbal and written form.

Responsabilités

• Provide quality care in the Clinical Nurse Liaison role to patients from diverse backgrounds.
• Develop relationships with referral sources to enhance admission processes and provide community support.
• Conduct educational in-services with medical staff or community groups on hospice topics.
• Coordinate with external medical staff and referral sources to determine patient eligibility for hospice or palliative care.
• Assist with transition of care and admissions to inpatient centers, home hospice, or facility-based services.
• Complete documentation for new referrals and follow-ups after each visit.
