Job summary

Novant Health Hospice is seeking a Hospice Liaison Nurse at Rowan Medical Center in Salisbury, NC. This hospital-based role coordinates referrals, conducts clinical assessments, facilitates admissions, and guides transitions from acute care to hospice services.

Qualifications

• RN licensure in the relevant state
• Bachelor's degree in nursing required
• BSN preferred and encouraged for advancement

Responsibilities

• Process incoming hospice referrals and coordinate admission scheduling
• Conduct comprehensive clinical assessments to determine hospice eligibility and patient needs
• Facilitate hospice admissions and ensure smooth transitions from hospitals to hospice
• Collaborate with physicians, case managers, discharge planners, and interdisciplinary teams
• Educate patients, families, and referral sources on hospice services and eligibility
• Support discharge planning and help remove barriers to timely hospice placement
• Build and maintain relationships with hospital staff and community partners
• Ensure compliance with hospice regulations and organizational policies
• Advocate for patients and families to uphold comfort, dignity, and quality of life
• Coordinate care to ensure exceptional patient and family experiences during transitions
