Job summary

Advocate Aurora Health is seeking a Part-time Hospice RN to join the 34141 IL Hospice Divisional - Nursing: SCT team for after-hours weekend coverage. This role supports hospice patients at home and in community settings with compassionate care.

Responsibilities include nursing assessment, triage, symptom management, education for patients and caregivers, and coordination with the interdisciplinary team. Requires RN license, BLS, and hospice/home health experience.

Qualifications

• Completion of an accredited nursing program.
• One year of hospice nursing experience.
• Active Registered Nurse license in the state of practice.
• Ability to travel to patient homes and community settings.

Responsibilities

• Incorporate responsibility, accountability, critical thinking, and sound clinical decision-making into daily nursing practice.
• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms, provide education, and determine appropriate interventions.
• Conduct in-person visits to manage acute symptoms and ensure patient comfort and safety.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
• Document all patient encounters and communications in the EMR accurately and timely.
• Adhere to hospice policies, procedures, and regulatory requirements.
