Job summary

HopeHealth is seeking a Registered Nurse for its affiliate HopeHealth Hospice & Palliative Care. This position on our Home B team serves Cranston, Providence and Warwick, with a caseload and coverage as needed.

This is a part-time, 24-hours-per-week weekend role, with Saturday and Sunday 8:00am–8:30pm. The RN will assess, implement care plans, educate patients and families, and coordinate with the interdisciplinary team to deliver compassionate end-of-life care.

Qualifications

• RN required.
• BSN preferred.
• 1–3 years of related experience.
• Strong nursing assessment, clinical skills and communication skills.
• Home care and/or related experience preferred.

Responsibilities

• Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living.
• Implements the individualized plan of care and revises the plan as necessary; plans for discharge/transfer as appropriate.
• Educates patient/family/caregiver regarding the disease process; teaches self-care techniques as ordered by the physician.
• Initiates preventive and rehabilitative nursing procedures.
• Coordinates all patient/family services with the interdisciplinary team to develop and revise the care plan.
• Informs the physician and others of changes in patient needs and outcomes; updates the plan of care.
• Determines scope and frequency of services based on acuity and needs.
• Coordinates care with family/caregiver and other disciplines/services.
