Job summary

Four Seasons is seeking an RN to provide nursing coverage to patients, families and community partners through phone and in-person visits, including post-admission care. The role requires care within regulatory standards, effective patient communication, and coordination with the care team.

Responsibilities include nursing assessments, care planning, education, and collaboration with other disciplines to ensure high-quality hospice services, with a 36-hour weekly night/weekend schedule in

Qualifications

• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
• HIPAA privacy rules and regulations.
• Knowledge of electronic medical records systems.
• Demonstrate excellent written and oral communication skills.

Responsibilities

• Provide nursing coverage to patients and families via phone and in-person visits.
• Assess the patient's health status and response to treatment.
• Record and report nursing assessments and patient responses.
• Collaborate with other health care providers to plan patient-centered care.
• Teach, supervise and coordinate nursing tasks and programs.
