Job summary

Four Seasons Hospice is seeking a Registered Nurse to provide nursing coverage for patients and families in Buncombe County and surrounding areas. Responsibilities include after-hours triage, admissions, and coordinating with case managers to implement patient-centered hospice plans.

Qualifications

• Two years of clinical experience preferred.
• Valid NC RN license required.
• Valid Driver’s License and proof of automobile insurance.

Responsibilities

• Provide nursing coverage to patients, families, and community partners by phone and in-person visits.
• Serve as first responder for after-hours symptom-based calls.
• Coordinate with case managers to implement patient-centered hospice plans.
• Assist with admissions and emergency admissions as needed.
• Record nursing assessments and report patient responses.
• Plan, initiate, deliver, and evaluate nursing acts per physician orders.
• Teach and supervise staff applying the treatment regimen.
• Collaborate with health care providers for the care plan.
• Ensure safe and effective nursing care and compliance with regulations.
