Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking experienced RNs for dedicated weekend hospice visits in the Twin Cities Metro. You will deliver one-on-one patient care with focus on comfort and family support.

Qualifications

• Must hold an active Minnesota RN license.
• Experience in hospice, ICU, ER, oncology, med-surg, home health, or palliative care highly valued.
• Strong assessment skills and ability to work independently.

Responsibilities

• Provide evening hospice nursing visits throughout the Twin Cities Metro.
• Assess patient needs and provide symptom management, medication administration, and comfort-focused care.
• Educate and support patients, caregivers, and family members.
• Deliver care in private homes, assisted living, memory care, and skilled nursing facilities.
• Collaborate with physicians and the interdisciplinary hospice team.
• Complete timely and accurate EMR documentation.
