Job summary

Pathways Healthcare is seeking a part-time Home Health Nurse to provide in-home care across Westwood and surrounding areas. The role supports preventive and rehabilitative nursing procedures on weekends, coordinating with physicians and a multidisciplinary team to deliver patient-centered care.

The ideal candidate has 2+ years of nursing experience, an active RN license, and proficiency with EMR systems. A valid driver’s license and reliable vehicle are required for travel across the Westwood

Qualifications

2+ years of nursing experience, ideally in geriatrics.
Proficient with electronic medical records systems.
Excellent bedside manner and communication skills.
Active BLS CPR certification.
Valid driver's license and own reliable vehicle.

Responsibilities

Initiates preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician.
Initiates and revises the plan of care as patient needs change.
Responds to afterhours calls during weekends.
Assesss patient needs through triage and in-home care when needed.
Collaborates with physicians and the care team to maintain a strong plan of care for each patient.
Responds to inquiries from patients, caregivers, and medical team.
Consults patients and caregivers before, during and after treatment.
