Job summary

Pathways Healthcare is hiring a part-time nurse to provide in-home nursing care in the Westwood area. The role covers weekends with a schedule of Saturday-Sunday, 8:30 am to 8:00 pm, serving Westwood and surrounding communities.

The ideal candidate has 2+ years of nursing experience, a nursing degree, and proficiency with electronic medical records. You will collaborate with physicians and a multidisciplinary team to deliver comprehensive care.

Qualifications

2+ years of nursing experience, preferably in geriatrics.
Degree in Nursing from an accredited institution.
Proficient with electronic medical records (EMR).
Excellent bedside manner and communication skills.
Knowledge of health care laws and regulations.

Responsibilities

Initiates preventive and rehabilitative nursing procedures.
Adminsisters medications and treatments as prescribed by the physician.
Initiates the plan of care and revises as patient status and needs change.
Responds promptly to afterhours calls (Weekends 8:30 AM to 8:00 PM).
Assesses patient needs through triaging and in-home care when needed.
Collaborates with physicians, nurse practitioners and interdisciplinary team to plan strong care.
Responds to inquiries from patients, caregivers, and medical team.
Consults patients and caregivers before, during and after treatment.
