Job summary

Pathways Healthcare is seeking a per diem registered nurse to provide in‑home care across Westwood and nearby communities. You will initiate preventive and rehabilitative nursing procedures, administer medications, assess patients, and develop individualized plans of care.

You will collaborate with physicians and the care team, supervise home health aides as needed, address questions from patients and families, and support discharge planning, while enjoying a flexible schedule, mileage

Qualifications

• Active nursing license in the state where the position is based.
• 2+ years nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and own a reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them.
