Pathways Healthcare is a leading home health and hospice organization guided by physician- and nurse practitioner-led care. Our Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.
We are seeking a part-time Home Health RN for weekend shifts in Boston, providing preventive and rehabilitative nursing, medication administration, and care planning in collaboration with the care team.
Qualifications
- 2+ years nursing experience, preferably in geriatrics.
- New nurses welcome with training program.
- Degree in Nursing from an accredited institution.
- Proficient with electronic medical records.
- Excellent bedside manner and communication skills.
- Working knowledge of health care laws and regulations.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable vehicle.
Responsibilities
- Initiates preventive and rehabilitative nursing procedures.
- Administers medications and treatments as prescribed.
- Initiates the plan of care and revises as patient status changes.
- Responds to afterhours calls on weekends (8:30 AM–8:00 PM).
- Assesses patient needs via triage and in-home care.
- Collaborates with physicians, NPs and interdisciplinary team for Plan of Care.
- Responds to inquiries from patients, caregivers and medical team.
- Consults patients and caregivers before, during and after treatment.
