Job summary

Pathways Healthcare in Boston seeks a part-time Home Health Nurse to join our physician- and nurse practitioner-led team. You will perform preventive and rehabilitative nursing procedures, administer meds, initiate and revise plans of care, and collaborate with physicians to deliver a strong Plan of Care for each patient.

The role requires 2+ years nursing experience (geriatrics preferred), a nursing degree, proficiency with EMR, a valid driver's license and vehicle, BLS CPR, and excellent

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• New nurses welcomed; training program offered.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assess patient's needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
