Weston County Health Services is seeking a Part-Time Registered Nurse to provide in-home health care, delivering compassionate, high-quality nursing services to residents in their homes. You’ll assess, plan, implement, and evaluate individualized care plans while documenting accurately and coordinating with families and providers.

The ideal candidate holds Wyoming RN licensure, ADN or BSN, and a current BLS certification, with a commitment to safety and patient education.

Qualifications

• Wyoming RN license required or eligible to practice in Wyoming.
• Current BLS certification or ability to obtain upon hire.
• Graduate of an accredited nursing program (ADN or BSN).

Responsabilités

• Deliver skilled nursing care in home health, including assessment, planning, implementation and evaluation of individualized care plans.
• Maintain accurate patient records, documentation, and required reports.
• Administer medications as ordered and monitor for side effects or adverse reactions.
• Monitor patient conditions and vital signs; respond to changes in condition.
• Coordinate care with providers, family members, and other departments for seamless service.
• Educate patients and families on care at home and safety measures.
