Qualifications

• Current RN licensure in the practice state.
• Graduation from an approved nursing program with at least two years of clinical nursing experience; hospice experience preferred.
• Knowledge of home health and hospice care frameworks and the nurse’s role within the care team.
• Understanding of nursing practice acts, agency policies, procedures, and reimbursement guidelines.
• Completion of 12 hours of continuing education annually.
• CPR certification.

Responsibilities

• Applying nursing theories and concepts to patient care.
• Documenting care in accordance with reimbursement guidelines.
• Coordinating physical care with patients, families, physicians, and the hospice team.
• Communicating effectively with patients, families, and agency personnel.
• Participating in interdisciplinary team (IDT) meetings and quality reviews.
