Job summary

Liberty Hospice in Roanoke Rapids, NC is seeking an experienced Hospice Licensed Practical Nurse (LPN) to join our compassionate team. This full-time role focuses on direct patient care, pain management, and coordination of services in a hospice setting.

You will document thoroughly, transcribe orders, participate in IDT meetings, and collaborate with RNs and care coordinators to ensure dignity and comfort for patients and families.

Qualifications

• Current LPN licensure in North Carolina.
• One year of clinical nursing experience, preferably in hospice or acute care.
• Ability to organize work independently and document thoroughly.
• Excellent communication skills (oral and written).
• CPR certification.

Responsibilities

• Provides direct patient care and coordinates services.
• Completes required documentation per policy and regulation.
• Follows plan of care developed by the RN.
• Coordinates hospice services with RNs, care coordinators, and staff.
• Transcribes physician orders as directed.
• Forwards notes to RN for care coordination.
• Attends IDT meetings.
