Beth Israel Lahey Health is seeking an Evening Hospice RN in Massachusetts. This role plans and delivers patient-centered hospice care, applying the nursing process with a focus on pain and symptom management, patient and family education, and collaboration with the interdisciplinary team.

The position is 25 hours weekly (paid for 32) with a Monday through Friday 4:00pm–9:00pm schedule. Full-time benefits apply, and Massachusetts RN licensure with BLS is required.

Qualifications

• Associate's Degree or BSN (preferred) from an accredited school of nursing.
• An active valid Massachusetts RN license.
• An active American Heart Association BLS certification.
• At least 1 year of direct care nursing experience, preferably in public health or home health care; EPIC experience is preferred.

Responsabilités

• Plan and deliver care using the nursing process (assessment, planning, intervention, evaluation).
• Interact with patients, families, and the inter-disciplinary team maintaining professional standards.
• Complete initial patient/family assessment to determine hospice care needs.
• Develop patient-specific care plans with goals based on nursing diagnoses and pain/symptom management.
• Provide instruction on medications, dosages, side effects, and administration.
• Document services using problem-oriented records; supervise Hospice Aides biweekly.
• Participate in hospice case conferences as required.
