Job summary

GBMC HealthCare Hospice is seeking a dedicated Hospice RN Weekender to assess and address patient and family needs, delivering nursing and supportive care to those at end of life. The role involves two 12-hour weekend shifts with eligibility for full-time benefits.

The ideal candidate will have an ADN/BSN, an active nursing license, and at least two years of nursing experience. Excellent communication, infection control, EMR proficiency, and a patient-family centered approach are required.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Two years of nursing experience, or equivalent combination of education and experience.
• Knowledge of universal precautions for infection control and safe practices.
• Strong time management skills and excellent interpersonal/communication abilities.
• Ability to be patient- and family-focused and to educate patients and families.
• Proficiency with EMR or ability to learn EMR.

Responsibilities

• Provides quality nursing care to patients with life-limiting illness to maintain the highest level of care in conjunction with the primary physician.
• Participates as a member of the interdisciplinary team to develop and implement a patient-directed plan of care.
• Evaluates patient/family response to interventions and modifies care plan accordingly.
• Educates patients and families to achieve skills for care, independence, and quality of life.
• Teaches infection control protocols to patients and families.
• Documents care accurately and timely in patient charts.
• Participates in Quality Improvement initiatives to improve patient care.
• Provides direction to Patient Care Partners, unit receptionists and volunteers.
• Completes daily acuity assessments for assigned patients.
• Attends to patient and family at the time of patient’s death.
