Job summary

Responsibilities

• Responds to requests in a positive manner.
• Gathers accurate information to initiate hospice and palliative consults.
• Coordinates referrals from physicians, hospitals, facilities and other sources.
• Schedules nurse practitioner visits and tracks appointments.
