Job summary

Beth Israel Lahey Health at Home is seeking a Hospice and Palliative Care Intake Associate to coordinate referrals and ensure smooth patient transitions into hospice services. Training is on-site at the Woburn location during weekdays.

Responsibilities include gathering patient information, scheduling NP visits, coordinating with care teams, and maintaining accurate documentation in the Epic EMR. Prior experience in health intake and strong communication are preferred.

Qualifications

• High school graduate required.
• Associates degree in healthcare-related field or business preferred.
• 1–2 years’ experience in a health-related field.

Responsibilities

• Responds to all requests in a positive manner.
• Gathers accurate information to initiate hospice and palliative consults.
• Communicates priorities to hospice and palliative teams.
• Schedules Face to Face visits with Nurse Practitioner within required timeframe.
• Uses Epic EMR to process and track referrals and maintain records.
