Job summary

Beth Israel Lahey Health seeks a Hospital Based Admissions Nurse to plan and deliver care for patients using the nursing process and to coordinate with hospital providers and the Hospice IDT. Required are an RN license in Massachusetts with Basic Life Support and a BSN preferred; minimum 3 years of hospice experience with inpatient hospice exposure is expected.

Qualifications

• Minimum 3 years Hospice Experience.
• Inpatient hospice experience preferred.
• Hospice or end of life care experience required.

Responsibilities

• Plans and delivers care using the nursing process (assessment, planning, interventions, evaluation).
• Coordinates care with hospital providers and Hospice IDT; attends IDT meetings as indicated.
• Performs daily nursing visits and assesses ongoing eligibility for GIP level of care; communicates with care team.
