Job summary

Beth Israel Lahey Health is seeking a Hospital Based Admissions Nurse to coordinate hospice admissions and determine GIP eligibility. You will work with physicians, case managers, and the hospice IDT to ensure timely access to hospice services and continuous patient care.

The role requires at least 3 years of hospice experience, an MA RN license, and strong communication and EMR skills; BSN preferred. On-site position with a patient-centered care focus.

Qualifications

• Minimum 3 years hospice experience.
• Inpatient hospice experience preferred.
• Hospice or end of life care experience required.

Responsibilities

• Describe hospice services to patient/family and respond to questions about care and costs.
• Coordinate care with hospital providers, IDT, and discharge planners.
• Perform preadmission activities and determine appropriateness of GIP level of care.
• Provide expert symptom management and coordinate with Hospice Medical Director.
