Job summary

Longleaf Hospice LLC in Atlanta, GA is seeking an Admission Nurse to conduct consultations and admissions visits, evaluating patient eligibility for hospice care in collaboration with the patient’s physicians.

You will assess medical status, plan and implement immediate care, and coordinate with the interdisciplinary team to develop and update the Plan of Care. Must have GA RN license, CPR, and a willingness to serve across the service area.

Qualifications

• Current license to practice as a registered professional nurse in Georgia.
• Current and continued CPR certification.
• At least two years of recent hospice experience.
• Excellent verbal and written communication and problem-solving skills.
• Valid driver’s license. Reliable, insured, automobile for work travel.
• Must be willing to complete admissions throughout Longleaf Hospice's entire service area.

Responsibilities

• Collaborate with the patient’s attending physician and the Hospice physician to determine clinical eligibility for hospice care.
• Assess patient’s medical status, plan, teach and implement immediate medical interventions on day of admission.
• Participate in coordination of all aspects of patient’s care with current professional standards.
• Participate in ongoing interdisciplinary comprehensive assessment as needed.
• Develop the initial Plan of Care (POC) and updates in collaboration with the IDG, physicians, and family.
• Contribute to patient and family counseling and education.
• Participate in QAPI and Hospice in-service.
• Coordinate medications and DME needs for same-day delivery.
• Complete all elements of admission process and first 24 hours of care.
• Communicate admissions information in care meetings for smooth transition with IDG staff.

The Admission Nurse performs consultation and admission visits to assess patient/family needs and collaborates with the patient’s attending physician and the Hospice physician to determine the patient’s clinical eligibility for hospice care.

RESPONSIBILITIES:

• Responsible for collaborating with the patient’s attending physician and the Hospice physician to assist him/her in determining the patient’s clinical eligibility for hospice care


• Responsible for assessing patient’s medical status, planning, teaching and implementing immediate medical interventions on day of admission, as prescribed by physician’s orders


• Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice


• Participates in ongoing interdisciplinary comprehensive assessment, as needed


• Develops the initial Plan of Care (POC) and updated POC, as needed, in collaboration with the interdisciplinary group (IDG), the patient’s attending physician, the Hospice physician, and the patient/family


• Contributes to patient and family counseling and education


• Participates in Quality Assessment Performance Improvement (QAPI) program and Hospice sponsored in-service


• Coordinates medications and durable medical equipment (DME) needs for same-day delivery


• Responsible for completion of all elements of admission process and first 24-hours of care


• Performs timely administrative functions relating to admissions, clinical records, Plan of Care, and mandatory abuse reporting requirements


• Communicates admissions information, issues and concerns in patient care meetings to assure smooth transition and continuity of care with other IDG staff, including a detailed report to the assigned Case Manager and other assigned disciplines


• Provides consultation and emotional/physical support to patients/families regarding Hospice services and disease processes


• Performs routine nursing visits when requested

Requirements
