Job summary

Peoples Hospice and Palliative Care is seeking a Hospice Admissions Nurse to perform consultation and admission visits, assess patient and family needs, and collaborate with physicians to determine hospice eligibility. You will finalize admission paperwork, develop the initial Plan of Care, and coordinate meds and DME for same-day delivery.

The role requires Florida RN license, CPR, and a valid driver's license with insurance; evenings, nights, weekends, and holidays may be required.

Qualifications

• Florida or multi-state RN license required.
• Two years RN experience preferred.
• Experience in acute, oncology, home health, and/or hospice setting preferred.
• CPR certification required.
• Valid driver’s license and auto insurance required.

Responsibilities

• Performs hospice consultation and admission visits to assess patient and family needs.
• Collaborates with attending physician and hospice physician to determine clinical eligibility.
• Completes admission assessment and Hospice Election of Benefit documentation.
• Develops initial Plan of Care (POC) with IDG and caregivers.
• Coordinates medications and DME for same-day delivery.
• Participates in after-hours and weekend on-call rotation.
• Maintains accurate records and submits reports on time.
