Job summary
Signature Healthcare at Home Hospice in the Portland, Oregon metro area is seeking a compassionate Registered Nurse (RN) for PRN Weekend Admissions. You will respond to hospice referrals, conduct admissions assessments in patients' homes or care settings, and coordinate education and plans of care with the interdisciplinary team.
This role requires autonomous practice, strong clinical judgment, and dedicated, patient‑centered care for patients and families during transitions.
Qualifications
- Current Oregon RN license in good standing.
- Minimum one year of nursing experience.
- Valid driver's license and reliable transportation.
Responsibilities
- Respond to hospice referrals during weekend coverage.
- Complete comprehensive hospice admission assessments in homes or care settings.
- Evaluate eligibility for hospice services per Medicare guidelines.
- Coordinate admission paperwork and clinical documentation.
- Develop and initiate individualized plans of care.
- Educate patients and families about hospice philosophy, services, medications, and resources.
- Collaborate with physicians, nurses, social workers, chaplains, aides, and other team members.
- Communicate with referral sources, hospitals, and community partners.
- Ensure timely documentation within the EMR.
- Provide compassionate support during transitions.
