Job summary

Cleveland Clinic Health System is seeking a Homecare Float RN for Weekender Flex in Independence, OH. You will provide direct nursing care in patients' homes, covering uncovered admissions and complex cases with 12-hour shifts from 10:00am to 10:00pm, weekends plus one additional weekday.

Responsibilities include wound care, labs, antibiotics education, and ensuring seamless care transitions while upholding quality standards and chart accuracy.

Qualifications

• Graduate of an accredited school of nursing (Diploma, ADN or BSN).
• Current RN license in the State of Ohio.
• BLS certification through AHA or Red Cross.
• Valid Ohio driver's license at hire; per policy thereafter.
• Proof of automobile insurance coverage at hire.
• One year of current RN clinical experience.
• One year in an acute medical-surgical or home care setting.

Responsibilities

• Provide direct nursing care per established policies and protocols.
• Monitor quality via chart audits and supervisory visits.
• Support quality improvement projects and department committees.
• Supervise starts of care, resumptions of care, and routine visits.
• Perform admission assessments and reassessments within time frames.
• Assess safety risks, abuse and neglect.
• Coordinate care across disciplines for well-coordinated patient care.
• Assess and document patients' pain and manage it.
• Maintain complete, up-to-date documentation at all times.
• Escalate changes in patient condition to physician as appropriate.
