Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary home care team. You will coordinate and provide direct and indirect patient care, perform skilled nursing assessments, and supervise aides in patients' homes across the service area.

This role emphasizes Medicare knowledge, care planning, and collaboration with physicians and insurers to ensure quality outcomes. A sign-on bonus of 7,500 is available for external candidates.

Qualifications

• Certifications: IL RN license, BLS 30 days, Driver's License
• Education: College Diploma Nursing
• 1 year nursing experience

Responsibilities

• Performs OASIS/485 changes within 24 hours of receiving them.
• Demonstrates understanding of Medicare Home Care benefits and coverage.
• Coordinates care with MD, care coordinators, and insurers.
• Supervises Home Health Aide and directs LPNs in home visits.
• Documents patient care and ensures timely transmission of orders.
