Qualifications

• RN license issued by the state in which you practice.
• BLS for Healthcare Providers certification required on hire.
• Driver license in Wisconsin or Illinois.
• Completion of an accredited nursing program.

Responsabilités

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts comprehensive nursing assessments and updates the care plan.
• Communicates with physicians to confirm and update treatment plan.
• Monitors patient response to treatments and revises care plan.
• Collaborates with RN Case Manager for discharge planning.
• Documents patient records accurately according to guidelines.
• Communicates problems to physicians and RN Case Manager with clear plans.
• Follows policies and updates to them.
• Performs patient positioning and transporting with proper technique.
• Demonstrates age-specific nursing knowledge.
