Advocate Health is seeking a part-time Registered Nurse to perform nursing duties on a 24-hour per week schedule, with shifts Fridays, Saturdays, and Sundays (8am-4pm or 8am-8pm on weekends). Night on-call rotation and weekend holiday coverage apply.

Requires RN license in Wisconsin or Illinois and BLS certification. Responsibilities include patient assessments, care plan updates, interdisciplinary collaboration, and discharge planning with RN Case Manager, in a facility serving the Wisconsin

Formación

• Registered Nurse license required (state of WI or IL).
• BLS certification required on hire, unless exempted by department leader.
• Valid driver license with access to a vehicle.

Responsabilidades

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts comprehensive patient assessments and updates care plans accordingly.
• Communicates with physicians to update treatment plans and relays changes to RN Case Manager.
• Monitors patient responses to treatments and revises care plans as needed.
• Aids discharge planning with RN Case Manager and coordinates with community resources.
