Job summary

Advocate Health in Wisconsin is seeking a part-time Registered Nurse to perform scheduled visits across the Milwaukee metro area. You will assess patients, update care plans, coordinate with physicians and RN Case Managers, and document accurately.

Flexible weekend coverage and on-call rotation are part of the program, with opportunity for incentive pay and career growth. You must have a valid RN license, BLS, and a nursing education program completion.

Qualifications

• Proficiency in clinical nursing skills and judgment under supervision.
• Strong written and verbal communication with patients, families, and staff.
• Ability to build rapport with diverse patients and colleagues.
• Capable of making critical, independent decisions when needed.
• Good organization, prioritization and problem-solving abilities.
• Ability to multi-task with frequent interruptions.

Responsibilities

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts comprehensive physical, psychosocial, and environmental assessments.
• Updates care plans based on visit findings and care plan guidance.
• Communicates with physicians to update treatment plans with the RN Case Manager.
• Provides ongoing assessment of patient responses and revises care as needed.
• Participates in discharge planning with RN Case Manager and community resources.
• Documents patient records accurately per guidelines.
• Communicates problems, plans, and goals clearly to physicians and RN Case Manager.
• Follows policies and monitors changes; performs other duties as assigned.
