Advocate Health is seeking a registered nurse for full-time daytime coverage in the Milwaukee South corridor. You will conduct visits, assess patients, and update the care plan in coordination with physicians and the primary nurse.

The role requires strong communication and clinical skills to support discharge planning and ongoing care. The position emphasizes adherence to care standards, documentation accuracy, and patient safety while working within a dynamic hospital setting and on-call

Formación

• Registered Nurse license issued by the state where you practice.
• BLS for Healthcare Providers within 6 months unless not required by leadership.
• A valid driver’s license or Illinois license.
• Associate’s Degree in Nursing.

Responsabilidades

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts comprehensive physical, psychosocial, and environmental assessments and updates care plans.
• Communicates treatment plan updates to physicians and the primary RN.
• Monitors patient responses and updates care plans as needed.
• Schedules patients and conferences with the primary RN.
• Assists with discharge planning and continuity of care after discharge.
• Documents patient records accurately per guidelines.
• Communicates problems and plans clearly to physicians and RN.
