Job summary

Advocate Health in Milwaukee seeks a part-time registered nurse to perform scheduled visits and collaborate with the care team to assess patients, update plans, and ensure continuity of care. The role involves on-site duties with holiday rotation and night on-call schedule.

You will document care plans, communicate changes to physicians, and support discharge planning, while adhering to safety and policy guidelines.

Qualifications

• Registered Nurse license in the practicing state.
• BLS for Healthcare Providers certification required on hire.
• Valid driver's license.

Responsibilities

• Perform scheduled visits and coordinate with the interdisciplinary team.
• Assess physical, psychosocial and environmental factors; update care plan.
• Communicate plan updates to physicians and RN Case Manager.
• Monitor patient response and revise care plans as needed.
• Assist discharge planning with community resources for continuity of care.
• Document patient records accurately per guidelines.
• Refer problems to physicians and RN Case Manager with clear action plans.
• Follow policies; adapt to changes; ensure safety during patient handling.
