Advocate Health in the Milwaukee metro area is seeking a part-time registered nurse to perform scheduled visits and collaborate with the interdisciplinary team. You will assess patients, update plans of care, and communicate changes with physicians and the RN Case Manager.
The role requires an active RN license, BLS certification, and about 1 year of nursing experience. This position includes weekend coverage and night on-call requirements with weekend program pay.
Formación
- RN license required to practice in the state.
- BLS certification required or to be obtained on hire.
- Typically 1 year of nursing experience.
- Proficiency in clinical nursing skills and judgment.
- Excellent communication and interpersonal abilities.
Responsabilidades
- Perform all scheduled visits and coordinate with the interdisciplinary team.
- Conduct comprehensive patient assessments and update care plans as needed.
- Communicate treatment plan changes to physicians and RN Case Manager.
- Monitor patient responses to treatments and document progress.
- Assist with discharge planning and coordinate post-discharge resources.
