Optum’s Geisinger Home Health team is hiring a Registered Nurse to provide and direct nursing care in patients’ homes, coordinating the plan of care with physicians and the care team. You will guide care, ensure quality, and support family engagement in a collaborative environment.

The role offers a $10,000 sign-on bonus for full-time hires, mileage reimbursement, and flexible scheduling. Requirements include current RN license, 1+ year home health experience, CPR, and a valid driver’s license

Qualifikationen

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current Driver’s License and access to reliable transportation.
• Current CPR Certification.
• Ability to work in diverse home environments.

Aufgaben

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and health care team.
• Supervises home health aides and licensed nurses, providing instruction and task assignment per regulations.
• Performs supervisory visits and ensures high-quality clinical services within plan of care.
• Completes comprehensive assessments (OASIS) and documents visits per policy.
• Collaborates with physicians and staff to ensure continuity of patient care.
