Job summary

Geisinger Home Health is seeking a Registered Nurse to provide and direct nursing care in patients' homes, coordinating the total plan with physicians and other health care professionals. The role supports implementation of the plan of care and requires current RN licensure and CPR certification.

You will work in a flexible schedule, with mileage reimbursement and local coverage in Luzerne County, PA. Sign-on bonuses are available for full-time external hires and a comprehensive benefits package

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR Certification.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Documents and delivers high-quality clinical services within the plan of care and infection control standards.
• Completes comprehensive assessments (OASIS) including medication reconciliation.
• Documents patient visits per policy and payer requirements, and aligns with agency policy.
