Job summary

UnitedHealth Group is seeking a Registered Nurse for Home Health to provide and coordinate care in patients' homes, guided by physician orders and agency policies. You will supervise home health aides, perform assessments, and ensure care continuity while collaborating with physicians and families.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license with vehicle or reliable transportation.
• CPR certification preferred or able to obtain within 90 days.

Responsibilities

• Initiates and revises the plan of care in collaboration with the physician and health care professionals.
• Supervises home health aides and LPNs, provides instruction, and assigns tasks per state and federal regulations.
• Provides supervisory visits and ensures compliance with infection control standards.
• Completes comprehensive assessments (OASIS) and documents patient visits per policy.
• Coordinates care with physician, agency staff, patient, family, and community resources.
