Job summary

UnitedHealth Group’s Geisinger Home Health division is seeking a registered nurse for home health services in Luzerne County, PA. This role offers a $15,000 sign-on bonus for full-time external hires, flexible scheduling, and mileage reimbursement.

You will provide and direct nursing care in patients’ homes as prescribed by physicians, coordinate care with the care team, and ensure compliance with regulations and agency policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current Driver’s License and access to a dependable vehicle.
• Current CPR Certification.
• Ability to function in any home setting with diverse clients.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to regulations.
• Provides supervisory visits as required.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately.
• Documents patient visits per policy and payer requirements, syncing with agency policies.
• Communicates with physicians, patients and families to ensure quality care and service excellence.
