Job summary

Optum is hiring a Registered Nurse in Home Health to deliver and direct nursing care in patients' homes as prescribed by physicians and compliant with laws and agency policies.

You will coordinate total care plans with an interdisciplinary team, conducting assessments and ensuring continuity of care with patients, families, and community resources. This role offers a 40-hour weekday schedule and comprehensive benefits.

Qualifications

• Current and unrestricted RN licensure.
• Active driver's license and access to reliable transportation.
• Current CPR Certification or ability to obtain within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Strong communication and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Documentation and care delivery within the plan of care and infection control standards; completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents patient visits per policy and payer requirements.
• Communicates with physicians, patients, and families to ensure quality care and service excellence.
