Optum’s HomeCall division seeks a Registered Nurse to provide and coordinate home health care for patients in their homes, following physician orders and regulatory standards. You will collaborate with physicians, agency staff, patients, and families to ensure continuity of care and optimal outcomes.

The role requires an active RN license and driving to patient homes. A $10,000 sign-on bonus is available for qualified external hires, with a comprehensive benefits package and a weekday schedule.

Formación

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle or public transportation.

Responsabilidades

• Initiates and revises the plan of care with the physician and health care team.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Performs comprehensive assessments (OASIS) including medication reconciliation.
• Documents visits per policy and payer requirements.
