ProHealth Home Health & Hospice in Dallas, TX seeks a Weekend Home Health RN to provide skilled nursing care in patients' homes on a PRN basis. This on-site role supports the agency's plan of care under supervision of the clinical leadership.

Ideal candidates are actively licensed RNs with at least two years of nursing experience, capable of independent initiative, and able to document care and communicate effectively with patients and families in English.

Qualifications

• Graduate of an approved school of professional nursing with active state license as registered nurse, or a multi-state license issued by an NLC member state.
• Ability to read, write, and comprehend English.
• Two (2) years’ nursing experience.
• Acceptance of philosophy and goals of this organization.
• Ability to exercise initiative and independent work.

Responsabilités

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Initiates the plan of care and necessary treatments.
• Provides those services requiring substantial specialized nursing skills.
• Initiates appropriate preventive and rehabilitative nursing measures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences.
• Coordinates and communicates changes in patient condition.
• Counsels patient and family in meeting nursing needs.
• Participates in inservice training and adheres to agency policies.
• Processes orders and notifies physician of patient needs and changes in care.
