Job summary

Frontpoint Health is seeking a Registered Nurse (RN) for Home Health visits in Texas. The RN will provide skilled nursing care, conduct patient assessments, and coordinate care with the interdisciplinary team. Responsibilities include managing the patient's clinical records and ensuring compliance with HIPAA.

The ideal candidate will have a current Texas RN license, over 2 years of experience in Home Health, and be certified in CPR. Reliable transportation and familiarity with EMR are essential for this role.

Qualifications

• Current Texas State license as a Registered Nurse.
• 2+ years experience as an RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Driver's License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (Kinsser preferred).

Responsibilities

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient's condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care.
• Provide proper patient education for home health care procedures and strategies.
• Responsible for the overall clinical record maintenance.
• Maintain confidentiality of patient PHI through HIPAA law.
• Keep the patient comfortable through palliative care.
