ProHealth Home Health and Hospice is seeking a Weekend Home Health RN to deliver skilled nursing care to patients in their homes in the Fort Worth area. This part-time, on-site role supports patient assessments, care planning, and coordination with the care team under supervision of clinical leadership.

The RN will perform initial and ongoing evaluations, administer physician-ordered therapies, educate patients and families, and document all visits with accurate progress notes.

Formación

• Active RN license or multi-state license via Nurse Licensure Compact (NLC).
• Ability to read, write, and comprehend English.
• Two years of nursing experience.
• Acceptance of the agency's philosophy and goals.
• Ability to exercise initiative and independent judgment.

Responsabilidades

• Provides services according to the plan of care and physician orders.
• Performs initial evaluation and regular reevaluations of patient nursing needs.
• Initiates and adjusts the plan of care as required.
• Delivers skilled nursing interventions and preventive care.
• Completes timely clinical notes and progress summaries for each visit.
