Job summary

CVHCare, based in San Ramon, CA, is seeking a Weekend Home Health RN to monitor patient health and develop care plans. This role requires a minimum of 1 year of RN experience, an active CA nursing license, and reliable transportation. The position offers both full-time and part-time schedules, benefits including medical, dental, and vision, and is not remote. Training is onsite for 5 days prior to starting work.

Qualifications

• Minimum 1 year RN experience, preferably in acute, SNF, Home Health or Hospice setting.
• Active CA Registered Nursing License.
• Reliable transportation.
• Current CPR or BLS Card.

Responsibilities

• Monitor patient’s overall health and develop an individualized plan of care.
• Function as a case manager and work independently to deliver quality healthcare.
• Provide specialized services such as wound therapy, post-surgical care, and ostomy care management.
