Job summary

Carle Health in Illinois is seeking a Home Care RN/Field Case Manager to coordinate and provide skilled nursing care across multiple counties. You will work independently as part of an interdisciplinary home care team, performing assessments, planning care, and educating patients and families.

This role includes complex wound care, PICC/Hickman line care, IV infusions, central lines, and supervision of Home Health Aides.

Qualifications

• IL RN license and BLS within 30 days.
• Driver's license and proof auto insurance.
• College Diploma Nursing with 1 year nursing experience.

Responsibilities

• Coordinate and provide direct and indirect patient care per plan of care.
• Perform OASIS assessments for admissions, recertifications, resumptions and discharges.
• Manage complex wound care including wound vacs, dressings, and IV infusions.
• Supervise Home Health Aide and coordinate care with the health care team.
• Communicate with physicians, care coordinators, and insurers regarding patient updates.
