Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate patient care across a broad home-based scope in central Illinois. You will perform comprehensive nursing assessments, manage wound care (including wound vacs and PICC line care), and collaborate with physicians, nurses, and other clinicians to ensure high-quality outcomes for patients in their homes.

The role emphasizes independent practice with an interdisciplinary team, ensuring documentation accuracy and regulatory

Qualifications

• IL RN license required.
• BLS certification required.
• College diploma in nursing; ~1 year nursing experience beneficial.

Responsibilities

• Perform OASIS assessments for admissions, recertifications, resumptions and discharges.
• Coordinate direct and indirect patient care using the nursing process.
• Manage complex wound care, including wound vacs and drains.
• Supervise Home Health Aide visits and coordinate with the health care team.
• Ensure timely documentation and review physician orders with the MD office.
