Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate patient care across central Illinois. You will work with an interdisciplinary team to deliver direct and indirect care in patients' homes, using the nursing process to meet physical, psychosocial, environmental, and spiritual needs.

Responsibilities include complex wound care, PICC line care, central line management, IV access, and ensuring timely documentation, admissions, and care conferences.

Qualifications

• IL RN license with active status and BLS certification.
• 1 year nursing experience in home care or related field.

Responsibilities

• Coordinate and provide direct and indirect patient care using the nursing process.
• Perform complex wound care including wound vac and dressings.
• Monitor central lines including PICC and Hickman catheters; perform dressing changes.
• Access mediports for IV infusions and blood draws; monitor and manage IV therapies.
• Lead home health aide supervisory visits and coordinate care with the health care team.
• Ensure timely admission, documentation, and discharge processes as per policy.
