Job summary

VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead Starts of Care and coordinate patient transitions during weekends. This field-based role offers clinical autonomy and the opportunity to deliver high-quality care for patients in their homes.

As the clinical lead, you will assess, plan, and implement care while educating families and coordinating with physicians. A valid RN license, CPR, and at least 1 year of home health or related experience are required.

Qualifications

• Active RN license and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
