Job summary

VitalCaring is seeking a PRN Weekend Home Health Registered Nurse to lead Starts of Care and transitions home for patients in the weekend. You will work autonomously in patients' homes, conducting assessments, developing care plans, administering skilled nursing care, and coordinating with physicians and therapists.

The ideal candidate has 1+ year RN experience (home health preferred), an active RN license and CPR, and excellent communication.

Qualifications

• Active RN license and CPR certification required.
• Minimum 1 year RN experience, home health preferred.
• BSN or higher; graduate of accredited nursing program.

Responsibilities

• Conduct in-home assessments and develop care plans with physicians and care teams.
• Deliver skilled nursing care including medications and treatments.
• Coordinate across therapists, aides, and physicians to manage transitions and outcomes.
• Maintain timely, accurate EMR documentation and supervise aides.
• Participate in on-call rotation as appropriate.
