Job summary

Senior Care Solutions is a Medicare-certified home health agency seeking an experienced RN for Baylor Weekend Home Health across the West Palm Beach to Delray Beach service area. You will work independently in patients' homes with support from Clinical Managers, QA, and office staff.

As a Baylor Weekend Home Health RN, you will perform SOC/ROC/Recert/Discharge visits, complete assessments, manage medications and wounds, document in EMR, collaborate with physicians and therapists, and educate

Qualifications

• Active Florida RN license is required.
• 1 year Medicare home health experience preferred.
• OASIS experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform SOC, ROC, Recertification, and Discharge visits.
• Complete comprehensive assessments and develop individualized plans of care.
• Provide skilled nursing services including med management and wound care.
• Accurately complete OASIS and clinical documentation in EMR.
• Collaborate with physicians, therapists, and interdisciplinary team.
• Educate patients and caregivers to promote safety and independence.
• Adhere to Medicare, ACHC, and agency policies.
